A program that rolled out in Hasting’s County earlier this year is now starting for its neighbour, Lennox and Addington County.
Daryl Kramp, MPP for Hastings-Lennox and Addington, says the program will work with primary and community care providers to make sure eligible seniors have 24/7 support. This includes non-emergency home visits, in-home testing, monitoring for those who have chronic medical conditions, and several other programs.
The program was introduced to Hastings County in February of this year. Early data says that it has reduced hospital admissions in the area. The program is fully funded by the provincial government as a part of its push to improve long-term care.
